Grace Episcopal Church frequently uses photographs and video to capture and share stories about parish life to its membership, guests, the diocese, and local community.

Grace Episcopal Church may use photographs and/or videos that may include your child/children in materials produced by or for the church, including but not limited to our bulletins, posters, brochures, newsletters and other printed and electronic materials and digital media (such as our website, Facebook, Instagram, Zoom). Grace Episcopal Church will not identify children age 12 and under by name or any other identifying information without prior parental consent. For and in consideration of my/our child being allowed to participate in all Christian Formation activities associated with Grace Episcopal Church, which includes Choir activities, beginning August 1, 2023, and ending July 31, 2024, the undersigned, being the custodial parent(s) or legal guardians(s) of the child above, do agree as follows:

1. To release Grace Episcopal Church, its agents, employees, and Christian Formation leaders and chaperones (“Grace Church”) from any and all claims arising out of program activities, other than actions arising out of the willful misconduct and grossly negligent actions of Grace Church.

2. To indemnify and hold Grace Church harmless from any and all claims that may be asserted against Grace Church by third parties arising out of program activities.

3. I/we acknowledge that there are inherent risks involved in any group activity, and this release/indemnity agreement is signed freely and voluntarily after review.

4. In the event my child’s participation in any program activity is terminated early because of illness and/or discipline problems, I/we understand that I/we will be responsible for special transportation expenses for my child’s early departure, but I/we will be notified prior to such arrangements being finalized.

The parties agree that the electronic signature of a party to this Agreement shall be as valid as an original signature of such party and shall be effective to bind such party to this Agreement. The Parties agree that an “electronic signature” may consist of a party typing his/her name in a signature block. The parties agree that any electronically signed document (including this Agreement) shall be deemed (i) to be “written” or “in writing,” (ii) to have been signed and (iii) to constitute a record established and maintained in the ordinary course of business and an original written record when printed from electronic files. Such paper copies or “printouts,” if introduced as evidence in any judicial, arbitral, mediation or administrative proceeding, will be admissible as between the parties to the same extent and under the same conditions as other original business records created and maintained in documentary form. Neither party shall contest the admissibility of true and accurate copies of electronically signed documents on the basis of the best evidence rule or as not satisfying the business records exception to the hearsay rule.
